Behavior history forms should be filled out on your COMPUTER.  Data entry is to be completed by the actual pet owner or adult legal guardian and not a third party.  The form may take up to an hour to complete. You may save and resume completing the form as needed.  In person appointments are only available in the Central Texas region.  Outside of our area, audiovisual consultations may be available directly with the pet's regular veterinarian.  Upon receipt of the completed history form, our office will contact you within 48 hours to schedule your pet's behavior consultation. If you have not heard from us within a 48 hour period, please contact us to verify we have received your email. Behavior consultations will not be scheduled without receipt of a completed history form.  Please contact us if you have any questions regarding history forms. 

To confirm initial behavior appointments, we will request a $150 non-refundable deposit at the time of scheduling.  The deposit is applied to the actual consultation fee. Video consultations with veterinarians are payable in full at the time of scheduling. 

NOTE: The $150 deposit is non-refundable because case preparation and file review begins shortly after scheduling. A significant about of work on your case in preparation occurs prior to the actual behavior consultation.

Veterinary referral forms should be filled out and submitted by your pet's regular veterinarian.  A copy of your pet's medical records can be emailed or faxed from your pet's regular veterinarian to: info@veterinarybehavior.com or 1-888-550-3321.
